A loot radar mod like animal instinct will help you find them since they appear as a blue blob on the minimap. The fragments don�t spawn in fixed locations. Specifically, you will need to scan 223 fragments in total. You can buy the codex scanner from the equipment tab in the marketplace. Cephalons fragments have a 100% chance to spawn, you just have to find them. Each planet has either two or three pieces of lore requiring you to scan either 3, 5, or 7 fragments to unlock them. Before this i had found 2/3 of these on mercury and 2 on earth already so i knew they spawned in random locations on the map.

The only requirement i have left is to scan two more cephalon fragments, but they�re proving to be elusive.
